There is a small round soft tissue mass lateral to the malleus on the bottom left image.
Note that the head of the malleus is displaced medially by this mass.

The direct coronal images show that the mass is confined to Prussak's space and
there is slight erosion of the adjacent scutum. There is demineralization or erosion
of the malleus and incus. The left tympanic membrane is also thickened and retracted.
The right ear is normal.
