Sagittal T1-WI demonstrates a large mass in the right neck that is mostly isointense to
surrounding muscle. The axial T2-WIs with fat saturation demonstrates heterogeneous
increased signal and edema. Note the absent flow void and intermediate signal within the right
internal jugular vein which is filled with thrombus. Edema extends into the right carotid space
although the right carotid remains patent. The airway and esophagus are deviated leftward.
Legend
Post Gadolinium demonstrates the multiloculated collection with intensely
enhancing septations. Again note the enhancing thrombus in the internal
jugular vein and the extension into the sternocleidomastoid muscle.
Also note the worrisome extension into the upper mediastinum.
