Axial FLAIR demonstrates significant reduction in caliber of the right cavernous carotid.
Note the effacement of the right occipital cortical sulci with associated increased signal.
There is chronic appearing deep white matter disease, probably ischemic demyelination.
No flow is evident in the distal right cervical carotid. Source images from a TOF MRA
demonstrate both reduced size and signal of the right cavernous carotid. The MIP and
collapsed images demonstrate no flow in the right internal carotid artery. The right MCA
receives collateral flow through a patent A-com.
There is both restricted diffusion and decreased perfusion in the right occipital cortex.
Note that the dip in the perfusion curve is the normally perfused brain with the loss of
signal secondary to the T2 susceptibility caused by gadolinium flowing into the target area.
The nonperfused ROI remains flat.
