On axial T2-weighted images, a heterogeneous mass is demonstrated in the expected region of
the pineal gland with marked high signal extending into the adjacent white matter bilaterally
(left > right) of the midbrain, thalami and temporal lobes.

Axial FLAIR image (left) shows superior extension of the abnormality. A sagittal T1 image
(right) shows the mass occupying the expected region of the pineal and quadrigeminal cisterns,
completely obliterating the cerebral aqueduct, tectum and compressing the midbrain, cerebellar
vermis, and splenium of the corpus callosum. The fourth ventricle is slightly effaced but
otherwise appears uninvolved.
