Post contrast axial CT images at the level of the oropharynx demonstrate soft tissue
swelling overlying the left parotid gland. The left parotid gland is expanded and
partially replaced by intermediate attenuation material, with involvement including
the deep lobe of the parotid. A rim of contrast enhancement surrounds this collection.
The retromandibular vein is displaced medially and the left mandibular condyle anteriorly.
Legend
Follow up images show greater anterior extension of the lesion with reduced attenuation
centrally. There is now clearly a mixture of gas and fluid expanding the left parotid.
Inflammatory changes extend anteriorly to involve the masseter muscle, as well as the
sternocleidomastoid muscles posteriorly. There is also effacement of the parapharyngeal
fat at the left oropharynx. Rim enhancement is more discrete. The left common carotid
artery and jugular vein remain patent.
