Lt two images: Sagittal T2 (with fat saturation) and T1 weighted images of the
lumbar spine reveal diffuse patchy abnormal marrow signal, low on T1 and
high on T2, in the bodies and posterior elements of the vertebrae.
Rt top & bottom images: Axial T1 weighted images show again the abnormal
low signal lesions involving the bodies, articular processes and spinal processes.
Lt image: Sagittal fat suppression post gadolinium image of lumbar spine
shows the previously noted marrow lesions enhance strongly with contrast.
Some epidural extension has occurred posteriorly at L4 and L5.
Rt top, middle & bottom images: Axial fat saturation post gadolinium images
demonstrate again the enhancing marrow lesions with extension to the para-
spinal muscles. Enhancing lesions are also noted in the adjacent ilium bilaterally.
